Have you been diagnosed with Chronic kidney disease (CKD), which has been classified as stage 3, 4 or 5?

Chronic kidney disease (CKD) is a long-term condition where the kidneys don't work as well as they should.

Have you been diagnosed with diabetes?

Diabetes is a serious condition where your blood glucose level is too high. There are two main types, Type 1 and Type 2.

Have you been diagnosed with hypertension (high blood pressure)?

Hypertension, also known as high or raised blood pressure, is a condition in which the blood vessels have persistently raised pressure.

Have you been diagnosed with Atrial fibrillation?

Atrial fibrillation is a heart condition that causes an irregular and often abnormally fast heart rate.

Have you been diagnosed with transient ischaemic attack (TIA)?

A transient ischaemic attack (TIA) or "mini stroke" is caused by a temporary disruption in the blood supply to part of the brain.

Are you currently being prescribed statins to lower cholesterol?
Have you previously been identified as having a 20% or higher risk of developing cardiovascular disease over the next 10 years?

This may have been in a previous health check or any other healthcare assessment through the health service in England.

This means that other activities will be taking place at many of our venues as well as that in some cases our practitioners will be lone working on site.
